The Jawbone Noise Shield Bluetooth Headset.
I really like mine. The design of the component parts is very well done and everything fits together very nicely -- snick snick, snick snick. I do feel like a dork wearing it, and vaguely antisocial at that, but I suppose that may change over time as more people wear these things.
What's absolutely stunning -- and what makes this device worthy of gnarly status -- is its amazing noise reduction technology. Check out the following demo video:

I love the Jawbone for all the reasons you and the video mention. Yet, I hate it for one very simple reason - bad design.
The earpiece for the Jawbone connects to the unit itself by a most fragile wire creating a clear single point of failure. Most people I know who have been using Jawbones for a while have broken them at this joint.
I worry that they were so focused on the 1) sound quality (which is truly mindblowing) and 2) the aesthetic appeal (they put one on display in the SFMOMA's collection) that they forgot to really QA these things and have actual users shove them in and yank them out of pockets for a spell.
